What is a Patient Portal?
A Patient Portal is a secure online website that gives patients convenient 24-hour access to personal health information from anywhere with an Internet connection using a web browser on your home computer, smartphone or tablet device.
What information can I find in the Patient Portal?
The information in the portal is from your Marshall Medical Centers’ electronic health record. Using a secure username and password, patients can view a summary of their personal health information including - lab results, allergies, medications, procedures and diagnoses.
Easy Enrollment Process!
Patients who are interested in signing up for the Patient Portal must first complete an authorization form. This form can be obtained in one of the following ways:
- Download and print by clicking on the button below, OR
- Obtain the form in the Health Information Management (Medical Record) department at either Marshall North or Marshall South, OR
- Mail upon request, which should be sent to email@example.com or firstname.lastname@example.org.
Get The Consent Form
The completed form should be signed and submitted with a copy of a picture ID. Please indicate on the authorization form whether you received medical treatment at Marshall North, South or both. It may be submitted to the Health Information (Medical Records) department to the mail address or either of the emails below.

Once your completed authorization is received, an email will be sent containing a link to the website. Click on the link to set up your personal Patient Portal. Use it to access your health information and pay bills online from anywhere you have Internet access.
