If contacted and considered to be a potentially qualified applicant, a comprehensive Application for Employement must be completed and signed.
It is a Medical Centers policy to hire employees who are drug free and tobacco free. Any applicant who declines to provide a specimen for drug/tobacco testing will not be considered further for employment.
I understand that the facility reserves the right to require its employees to submit to blood tests, urinalyses, or other screenings for alcohol, drugs, or tobacco products, or to allow inspection of bags (including purses or briefcases) or parcels brought into or taken out of the facility. I understand that refusal to submit to a urinalysis or blood test or other screening, when requested to do so, may result in termination of my employment.
Compliance with this facility's Substance Abuse policies (alcohol, drugs, and tobacco products) is a condition of employment. This hospital requires that every newly hired employee be free of alcohol, drug abuse, and tobacco products. Each offer of employment is contingent upon successfully completing a urinalysis test/screen for alcohol, drugs, and tobacco products in accordance with hospital policy. Continued employment is also contingent upon compliance with the hospital's Alcohol, Drug Abuse, and Tobacco Products Policy. This institution follows the "fair employment practice code" and there is no discrimination in the hiring of individuals based on sex, race, religion, age or physical or mental handicap unrelated to ability to perform the work required.
Please Note: A willful misrepresentation or statement concerning tobacco use on the application for employment shall be deemed grounds for dismissal, if the Medical Center is informed during employment that the employee/applicant has not been truthful.
